Active components of gel and their pharmacological action:

Metronidazole Benzoate:
Metronidazole – is derivation from nitroimidazole, has an antiprotozoal and antibacterial effect. The mechanism of effect of metronidazole is in biochemical interaction with DNA of a cell of microorganisms. It inhibits the synthesis of their nucleinic acids that leads to the death of bacteria.
Metronidazol suppresses anaerobic microflora of root canals very effectively, stops catabolic destruction of tissues blocking the inflammatory phenomena at the biochemical level. Along with it no allergic reactions or cases of accustoming to this preparation have been observed until today.

Chlorhexidine Diacetate:
Chlorhexidine is antiseptic. It has an antibacterial effect. The mechanism of action is the following: at high concentration of. Chlorhexidine cytoplasmatic contents of the bacterial cell precipitate and lead to death of bacteria. It is active to gram-negative and gram-positive microorganisms, yeast, dermatophytes and lipophilic viruses is active concerning a wide range of vegetative forms. It is especially effective to aerobic microflora of root cannals.

Hydrocortisone acetate:
Hydrocortisone acetate belongs to the group of glucocorticosteroids of a natural origin. It has antishock, anti-toxic, immunosupressive, antiexudative, anti-itch, anti-inflammatory, desensitizing, antiallergetic actions. It slows down reactions of hypersensitiveness, proliferative and exudative processes in the inflammation center.

INSTRUCTION

Method of application:

First visit:

a) Remove carious dentine and remains of the pulp from the pulp camera; wash out.

b) Isolate tooth from the saliva.

c) Carry out the primary tool processing of the root canal. It is desirable to use endolubricant on the basis of EDTA (for example Chela-Jen Gel). In case of using sodium hypochlorite during the tool processing it is necessary to finish the processing by washing the canal with a large amount of distilled water or physical solution. Dry up the canal with the help of paper pins. Do not use air!

d) Fill the canal (processed in the described way) with the help of the cannula (goes complete with the material) or the canal-filler until it doesn’t start going beyond the pulp camera.

e) Place a rolled ball of sterile cotton wool into the tooth cavity and make a temporary cement filling.

Second visit:

It takes place 2 – 3 days later after the first visit (as always, the tooth should be isolated from saliva).

f) Wash thoroughly with warm water to remove the gel remains from the canal.

g) Carry out the final processing of the canal: full mechanical and antiseptic processing, drying and sealing of the canals should be carried out.In some cases there is need to reuse the gel in the root canals under a tight hermetic bandage (duration of the treatment can be 1 – 3 days).

CLINICAL EXAMPLE OF USE OF MATERIAL JEN-METROHECOR:

Practicing doctor-dentist of the highest category, lecturer at IS NMAPO named after Shupik, Kiev.

Patient K, 36 years old. Chronic granulomatous periodontitis of the tooth 2.2 at the stage of exacerbation.

The previous treatment with the use of calcium hydroxide for temporary obturation of the root canal was not successful. The patient came to the clinic with complaints of pain in the area of the problem tooth and swelling of the soft tissues. It was decided to include the material Jen-MetroHeCor into the scheme of endodontic treatment. The material was placed in the root canal for 3 days. On the second day the patient confirmed that the pain disappeared and edema significantly decreased, which meant that the inflammatory process stopped. After 3 days, the treatment was continued according to the classical scheme. Constant obturation was performed by means of method of vertical condensation of the heated gutta-percha with the use of sealer AH Plus (Dentsply).

The treatment of 20 patients with a diagnosis “chronic granulomatous periodontitis” was performed with the use of Jen-MetroHeCor. The gel was applied for 1- 3 days (depending on the clinical situation) in the form of a medical intra-root bandage after medication and mechanical treatment of the infected root canal and then it was hermetically sealed with a temporary fillingIn all clinical cases the gel showed a high clinical efficiency and influenced a rapid reduction of inflammation in periapical tissues. This method of using Jen-MetroHeCor allows to avoid the reinfection of the root canals, which often occurs when the tooth left open and also allows to achieve positive results during shorter periods.
